Mit Demenz bist du krank!?


Heute mal ein weiteres Thema, das ich aus meiner Arbeit bestens kenne.
Demenz und auch Alzheimer. Beides sind herausfordernde Zustände, jedoch nicht das Gleiche oder Selbe. Unter dem Begriff der Demenz verbirgt sich eine Anzahl von Erkrankungen des Gehirns, die alle die Gemeinsamkeit haben, das es sich um Verluste der Speicherfähigkeit von Informationen betrifft. Dies kann einzelne Bereiche des Gehirns betreffen oder mehrere. 
So können Betroffene sich vielleicht Zahlen nicht mehr merken, oder eine Fremdsprache nicht mehr abrufen. Es kann aber auch sein, das vergessen wurde, wie man sich bewegen, oder essen soll. Während beim Allzheimer ein stetiger und allmählicher Abbau aller Bereiche vorzufinden ist. Alzheimer ist dadurch gekennzeichnet, sich nur in eine Richtung zu entwickeln. In die des Vergessens. 
Bei der Demenz ist kaum vorherzusagen, wie sich diese entwickelt. So kann sich der Zustand stetig verschlechtern, aber auch wieder für kurze Zeit verbessern. Es kann allmählich ein Abbau stattfinden oder es findet nur einmal einer statt und dann verbleibt die Person auf dem Niveau. Sowohl für Betroffene als auch Angehörige ist diese Realität oftmals schwer zu meistern. Denjenigen den es trifft kann es aus der Bahn werfen, sprichwörtlich. Ein Teil verschweigt es vor sich selber, bis es nicht mehr geht. Viele Kompensieren ihre Verluste sehr geschickt und sehr lange. Manche verändern sich im Wesen teils zu sehr unsympathischen Typen. Manch einer wird gar aggressiv, weil er nicht fassen kann was er meint zu wissen. Bedenkt man das viele sehr wohl mitbekommen, das ihr Gehirn nicht mehr tut was es bisher konnte. Es gibt auch viele die es nicht merken oder merken wollen und bis zum eisernen Ende beteuern, das sie geistig fit sind. Nun wird meist erst einmal mit Medizin reagiert und die Diagnose gestellt, mal mehr mal weniger feinfühlig. Oft landen, ja man muss es so nennen, die Betroffenen im Heim. Viele Heime sind personell nicht wirklich darauf ausgelegt und vor allem viele Pflegekräfte vergessen womit sie es zu tun haben, aber auch viele Angehörigen überschätzen ihr Können und die Kraft die es kostet, einen Betroffenen alleine zu betreuen daheim. 
Oftmals ist es einfacher als man denkt aber warum ist es überhaupt so schwer, der ganze Umgang? 
Hier setze ich mal eine provokative Behauptung an. 
Nicht der Begriff der Demenz ist problematisch oder das jemand die Diagnose Alzheimer bekommt- wo wir bis heute nur einen Bruchteil der Ursachen kennen- nein, ich sehe es in der Tatsache, das alle Formen der kognitiven, also der geistigen Einschränkung in diesem Bereich als Erkrankungen angesehen werden. Ich denke, statt diese Menschen als Krank anzusehen, was zum aggieren führt, sollte man es als einen Prozess betrachten, der einem Abschiednehmen gleicht, was zum begleiten führt. Die Menschen nehmen  bewusst, gewollt oder unbewusst Abschied vom Leben, so wie es war. Das ist ein Lernprozess. Betrachten wir es als Anghörige und Pflegepersonal auch aus dieser Perspektive, so lernen wir leichter damit umzugehen und nehmen auch bewusster Abschied. Ein langer Abschied zuweilen, aber eine wunderbare Chance. 
Sehen wir es von dieser Warte aus an, hören wir auf zu aggieren und reagieren statt dessen auf den Menschen und seiner Situation. Wir holen ihn dann dort ab, wo er gerade steht, anstatt ihn zu therapieren und an seinen eigentlichen Bedürfnissen vorbei zu gehen. 
Das ist nämlich, was immer und immer wieder passiert. 
Das ist was soviel Kraft kostet, diesen kranken Menschen zu behandeln, wo es nichts zu behandeln gibt. Ja, ich bin mir bewusst, das es Medikamente gibt,  die den Prozess hinauszögern können, wenn man sie sehr, sehr früh einsetzt. Ja, es sind auch junge Menschen betroffen, wo man bis heute nicht wirklich weiss, warum. 
Jedoch kann ich sagen, das viele im Alter nicht von ungefähr in das Vergessen geraten. Nicht nur unsere Ernährung und Lebensweise sorgt für diese Entwicklung, sondern auch, wen wir im Alter um uns herum haben. Wer alleine lebt, dem fehlt das geistige Austauschprogramm und wer im Heim mit klarem Kopf hineinkommt, wandern dennoch fast immer im Vergessen hinaus. Das kann aber nicht wirklich verwundern, wenn man seinen letzten Lebensabschnitt an einem Ort verbringen muss, das niemals ein Heim sein wird.  
Darum, sehen wir es als Abschied nehmen vom Leben, können wir den Menschen die Freiheit lassen so zu sein, wie sie sind und es gibt Projekte und Demenzhäuser, die genau das machen. Auf den Menschen reagieren, wann auch immer, wie auch immer und wo auch immer er steht. Dann ist die Pflege einfacher, der Umgang leichter und auch daheim würde man aufhören immer Angst zu haben und statt wie ein Hubschrauber über allem zu kreisen, dem Leben vertrauen und zum Beispiel die Mama eben drausen tanzen lassen wie ihr grade ist. Denn diese Betroffenen werden sehr häufig zu Kindern und bei denen reagieren wir auch statt zu aggieren, zu bestimmen oder nur zu fordern und zu verbieten. Am glücklichsten sind doch alle, wenn man einfach schaut, wo man steht. Kurzum, wenn wir aufhören Demenz in all ihren Formen und Alzheimer als blose Erkrankung zu sehen und statt dessen es mehr als einen Schritt im Prozess des Lebens, des Abschiedes nehmen, dann glaube ich, wird der Umgang miteinander wieder  menschenwürdiger werden, denn die Menschenwürde steht auch diesen Menschen zu.
Nicht zu vergessen das die Wahrnehmung als Krankheit oft den Menschen scheinbar die Freiheit gibt die Betroffenen ständig zu mahnen, ihnen Vorschriften zu machen, zu erklären was sie dürfen und was nicht, denn sie seien zwar krank, aber hätten sich doch gefälligst an die gesellschaftlichen Konventionen und Regeln zu halten. Gerade aber das können oder wollen diese aber nicht. Es wird den Betroffen damit die Vollwertigkeit als Mensch genommen. Krank ist gleich unvollständig und hat kein Recht auf eigene Meinung und Lebensgestaltung. 
Deswegen plädiere ich für das Umdenken und sage, es ist ein Teil des Lebensprozesses im Rahmen des Abschieds vom Leben selbst.

Dicke Mädchen/ Heavy Girls

Another movie to recommend and this movie is a low budget film. 
It's sure no "Hollywood" style, but very pure. 
Made in Berlin in Marzahn. It shows very pure and easy the way of living in east Berlin and so the colors are quite a bit unusual for most of us to watch in a movie.
But don't give up and wait a second when you see the movie on TV or DVD.... It's worth the waiting.
The movie is hard to put in a genre, cos it's funny but not comedy, it's sad, but not a sad film, it's earnest but not a dull movie. 
The film has some moments in silence and then others in action. The talkin' is very natural and the acting too. 

So this film is first of all a film about the simple live- live itself, then it is a film about a mother and her son, who is living with her, because she has dementia. At least the film is also the bitter sweet version of a love from two people coming up. 
What I liked is, that all the cliches and stereotypes where broken and you feel real good with this. It also shows that we may think about how we act with older people... and yeah with people who are not mainstream. 
The grandmother in the movie, the actress is actually 89 years old when she made the movie with her grandson. She is so lovely. 
The story has some turning and all of them are very good to follow, even you will be surprised but never be disappointed. 
I must confess that I smiled most of the time watching the movie and can only recommend it to watch for everyone, cos seems the movie has touched very different people in different countries where they showed the movie on festivals. 
So... a little but very much worth time out for you... except you only like the hollywood style, then this movie will maybe be to much for you...

Old Shoes.... what to do with them..?

Dementia

very common these days in the life of families... 
Not easy to deal with for everyone...
Some people kill themselves when they get the diagnose from the doctor...
Some people try to hide from that fact that they change in a way they never dreamed of...
Some people try to be open with this...
Some say you still can have a goooooooooood time with that.... can you? Is it true?
Alzheimer's organisations try to tell the people not to kill themselves cos still it's all worth living... Is it?
In the beginning for all people victim or not, it's more or less easy to deal with this, but then... the change comes, the challenge... and the question.. is this a god quality of Live? Is this a god Live? 
Oh yes it's true you have fine hours... moments... and if you're lucky, your healthy... The body can be save but the soul is starting to fly away, far away.. memories fades away... who are you then... another person... never the same... not knowing where to go, where to stay, what time it is or what's the place is called you are... not knowing what to do... live is getting boring..... very boring... you getting helpless and the things become more and more little.. just thinking of when to eat, when to sleep and when to shit... yes it's like that... very much.... is this worth those rare moments of peace you maybe have... 'cos for the most they comes a harder way with it... not only your mind is blowing away, your body is not the same.. you cannot trust your own body, you're ill, but don't know how to deal with it... helpless you are... Yes people keep watching over you, washes you, and feed you... at the end like a baby you are... Is this really what you want? And if it isn't what you want... Why do people say it is reprehensible to take you're own life when you decide not to live up to this... I mean... I see it at my work and know it from my family... the person you once loved and knew is gone but there is still the body and this body becomes a stranger to you too... Who is it you keep watching over then? The same? 
Well with dementia, no one can tell you how it goes...You can have luck and stay in a way you feel yourself good with but with Alzheimer -you know the way... It just goes down... down.. down... 

But one truth is: Most of Dementia and Alzheimer affected suffer in one or another way.... Why not then to say no to that and end your own life? 
No easy question.... but don't let people tell you it's an always god living with this illness... or there is a chance of god times with this... oh those chances are so little and this has nothing to do with our society, like some say... sure we should not drown those people away... when they need our help and understanding... but then..like I said... oh yes we can do to keep you as long as able in a god way... but this way ends someday too.. and then what... ? Victim or not.....?

Violence in nursing

Guess you have heard or read about this already... isn't it scary? Should never be but happens all the time and more then we think and believe.. not only at home when a daughter is psychologically harm her mother just with words and look. Most time we hear about this sad thing when they came to happen in nursing homes, where old people try to stay as good as they can till death comes and takes them home to heaven or so... No excuses... this should be or is the thinking of the most, but is it so easy? 
As a nurse do I know that one time, all of us nurses meet a patient, a old people who makes us angry and we have to fight hard not to harm them... you wonder why... well sometimes there seems no reason just... a feeling you get when you see someone.. like on the street, when you meet someone you directly know you don't like him... but on the street you can go on, change the sides..like that but when you are a nurse.. officially everyone has to be treat equal... no matter if you like him or her or not... well that's a god point and important to remember and keep in mind...  but actually in real life a big challenge sometimes... because those people you nursing are very near to you, not only physically also .. you know.. they come in your private zone... it's not comfortable to let in someone you don't want to, but we have to, it's part of the job.

But then... there is another side to look at...
What brings staff to harm someone, they never thought would do...
Those old people can act like children.. they can be lazy, don't listen to what you say or explain, can be stubborn, can lie to you, be mad at you without a reason, be offended because they have to do something they can but are to lazy to do... they can be dissatisfied with themselves but instead of seeing this they curse on you... and then you have not much time to work for them but some of them just "need" much more time then they really need -to do the things they have to do, but just because they feel like that... Or you try to give them positive advice but they refuse to take it, or you try to build them up, but they don't give a fuck about that... and all the sexual harassment you get of them... some know very well what they do (and I don't say it's a lie that old people have their needs but then... we are not family, not a friend or a lover for them... ).... yes the older they are the madder it can get.. Man and Woman, no difference... clapping, shouting, flirting, try to get a kiss from you, touching in every way they can... yes they try to touch your ass, your boobs.. or more intimate places... 
A lot to deal with and then too less time, to less stuff.... No wonder that it happens from time to time those sadly things, that people get harmed... sure no one wants that... but as long no one is fighting for a better healthcare, better pay for nurses, more stuff ... that long do we have to read about this...
